STATEMENT / AFFIDAVIT NEEDED FOR HIRE ACT


If you put a new employee on your payroll and want to enjoy a payroll tax holiday (no employer payments for Social Security taxes), the worker must certify to you that he or she has not worked for more than 40 hours during the 60 days prior to starting work for you.

The IRS has Form W-11 that you can use to obtain the employee's word that he or she is a qualified employee.
